1. Welcome and scope

Trophy Quest Farms is a hunting and guest operation owned by TQ Companies. In consideration of permission to enter and participate, the person named on this form agrees to these terms for the visit dates and authorized activities recorded below. TQ Companies, doing business as Trophy Quest Farms, is the operator under this agreement, not a travel consultant. Each hunter and non-hunting guest must have a separate form completed before participation. This agreement does not authorize an activity, replace a safety briefing, or change separate booking, payment, or refund terms.

2. Outdoor and property risks

Hunting and visiting the farm involve risks that may cause serious injury, death, or property loss. Examples include accidental firearm discharge, ricochets, arrows, and the conduct of other hunters; falls from tree stands, elevated blinds, ladders, steps, or uneven ground; equipment failure or improper use; vehicles and farm machinery; falling trees or limbs; wild animals, insects, ticks, or snakes; weather, heat, cold, lightning, and fire; water hazards; and slips, trips, or falls around the lodge and property. Rural conditions can delay communication, emergency response, and medical care. Non-hunting guests may also encounter these hazards. Activities involving vehicles or other equipment are permitted only when specifically authorized by staff.

3. Voluntary participation and safety

I will attend the safety briefing, ask questions, and participate only within my abilities and the activities staff authorizes. I understand and voluntarily accept the risks I know and appreciate. I may decline or stop an activity and will promptly report unsafe conditions, equipment defects, injuries, or concerns. I will follow staff directions, posted rules, lawful hunting regulations, licensing and hunter-education requirements, and applicable visibility requirements. I will handle firearms and bows safely, identify my target and what is beyond it, and keep firearms unloaded when staff or safety rules require it. I will use required fall protection for elevated stands, remain in authorized areas, and use equipment only after instruction. I will not hunt, handle weapons, or operate equipment while impaired by alcohol, drugs, fatigue, or medication. Unsafe conduct may result in removal from an activity or the property; separate booking terms govern financial consequences.

4. Fitness and emergency assistance

I will tell staff privately about limitations or assistance needs relevant to safe participation. I will not participate when unable to do so safely. If I am unable to communicate in an emergency, I authorize staff to request appropriate emergency assistance and contact the emergency contact listed below, subject to applicable law. I understand that emergency services and treatment may involve charges payable under applicable law and my coverage. This does not release negligent emergency conduct beyond the specific lawful adult release below or guarantee the availability of medical services.

5. Important adult liability release

PLEASE READ AND INITIAL. If I am an adult participant age 18 or older, to the fullest extent permitted by law, I release TQ Companies, doing business as Trophy Quest Farms, John Wilson, and the operation’s employees and guides acting within their farm duties from my claims for injury, death, or property loss arising from ORDINARY NEGLIGENCE in connection with the authorized visit and activities identified on this form. This includes ordinary negligence relating to premises conditions, equipment supplied by the operation, activity preparation, guiding, safety instruction, supervision, and emergency assistance. I understand that this limits my ability to recover damages for those ordinary-negligence claims. This release DOES NOT apply to gross negligence, reckless or willful misconduct, fraud, or any liability or right that cannot lawfully be waived. It does not waive booking refunds or obligations concerning payments. It does not release a minor’s claims. I have read the specific risks, had an opportunity to ask questions or obtain independent advice, and voluntarily agree to this limited adult release.

6. Minors: permission and acknowledgment

For a participant under 18, a parent or legal guardian must complete the minor section, provide permission, and arrange supervision consistent with staff requirements. The guardian acknowledges the described risks, agrees to applicable safety and conduct requirements, and confirms authority to give permission. A minor’s optional signature acknowledges the briefing and safety rules only. Neither the guardian’s signature nor the minor’s signature releases the minor’s injury claims under this form or requires the guardian to reimburse the operation for those claims. A guardian who also participates must complete a separate adult form for their own visit.

7. Agreement and records

This agreement applies to the identified visit, not unrelated future visits. Georgia law governs to the extent permitted by applicable law, preserving mandatory legal protections. Disputes may be brought in a court with lawful jurisdiction and venue. If a provision is unenforceable, the remaining lawful terms remain effective. Changes require a written agreement with TQ. No compulsory arbitration or shortened claim deadline is imposed. An adult signature, on paper or electronically, confirms informed agreement; for a minor it confirms guardian permission and acknowledgment only. TQ acceptance or countersignature is not a condition of the participant’s or guardian’s signed agreement. Staff must still confirm eligibility and authorize participation before any activity begins. An electronic record retains the exact terms, identifying details, signature, initials, account identifier, and signing time. You may print and retain a copy; TQ retains the form for visit administration and records.
